Cheat codes can only be unlocked by first beating the game on any difficulty level. (Yes, I realize this makes no sense. Sorry.) After completing the final level, the Cheat Codes sub-menu is accessible by selecting Rewards then Cheat Codes from the main menu.
Then unlock each individual code by completing levels in Time Trial mode as listed below.
Useful 100% Health Bug: Though not actually a cheat, it may be useful to note that Lara's health bar is refilled whenever the game reloads at a checkpoint. So if she's near death, try to hang on just until the next checkpoint. Then save and reload, and she'll be back at 100%.
